The papules on the nasal rim in this picture are characteristic of cutaneous sarcoidosis. Initial treatment should consist of steroids or other immunomodulators.

Acne excoriee, atypical mycobacterial infection, foreign body granuloma, Hodgkin lymphoma, lupus miliaris disseminatus faciei, and toxoplasmosis are considered on the differential of sarcoidosis but are not characteristic lesions as shown here.
